BEFORE YOUR VISIT
For new patients, be sure to bring your complete insurance information with you—including secondary insurance coverage, if you have it. For established patients, if there has been any change in your insurance, present the updated information to the office staff when you register.
Be prepared to make any necessary co-payments, according to your insurance plan, at the time of checking in. This can be made by credit card, check or cash.
Bring a list of your prescription medications with you to the office—include drug name, dosage, and how often you take it. Even if we already have this information, we will want to double check it with you as a precaution. This is only for prescription medications—you do not need to include vitamins, supplements, or over-the-counter medications (except do include aspirin, if taken routinely ).
Gather any information or test results that you want Dr Chow to review, and give them to the medical assistant when you check in.

If you have been hospitalized since your last visit, or had significant procedures or surgeries done, bring any summary information with you that you think Dr Chow should know.
If Dr Chow referred you to a specialist, please ask for a copy of their office note and bring it with you to your follow-up visit with us.
If wish to conduct the office visit in Mandarin Chinese, please let us know in advance.
Leave ample time for your office visit—on very busy days we can sometimes run behind schedule.
